How do I set-up a new Five Dysfunctions Team Assessment?

From the home page within the Administrator's Console, you can select the red icon that reads, “Team Assessment Setup.” The Team Assessment Setup page is where you create your assessment by entering the team's email addresses and customizing a message to participants.
